Why exhaustion persists despite “normal” labs — and how to decode what your body is really telling you.

 

The Puzzle of Persistent Fatigue

Nearly every patient with chronic illness names fatigue as one of their most disabling symptoms. This isn’t the “tired” that resolves with a nap. It’s bone-deep exhaustion that can leave someone struggling to get out of bed, string a sentence together, or even sit upright.

For many, routine labs return “normal,” leaving them dismissed with phrases like:

  • “Your labs look fine — maybe it’s stress.”
  • “Try exercising more.”
  • “It might just be depression.”

Yet research shows that unexplained fatigue affects millions worldwide, with chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME) estimated to impact between 10-20% of the global population (1). Long COVID has only magnified the problem (2).

The question patients ask is universal: Why am I so exhausted when all my tests are normal?

 

Rethinking Fatigue: The Cell Danger Response

To answer, we have to zoom in — to the level of mitochondria, the tiny powerhouses inside every cell.

Mitochondria do more than produce energy. They act as cellular sentinels, sensing safety and threat. When they detect toxins, infections, or oxidative stress, they intentionally downshift energy production — forcing the body into rest (3).

This phenomenon, called the Cell Danger Response (CDR), was elaborated by Dr. Robert Naviaux (4). In the CDR, mitochondria switch from “growth and energy” mode to “defense and protection” mode. Symptoms like fatigue, brain fog, and muscle pain are not malfunctions — they are adaptive strategies to slow the body down until safety is restored.

In other words: fatigue is a survival strategy.  I think this understanding is key to stopping the upset at our bodies when they don’t perform the way that we think they should.  In fact, I hope that this concept of fatigue as survival, can even allow for the possibility of feeling gratitude for your amazing body that knows how to help navigate these threats that it perceives!

 

Five Key Drivers of Chronic Fatigue

1. Mold and Mycotoxins

Mold exposure is one of the most common and under-recognized causes of fatigue. Mycotoxins injure mitochondria directly (5), generate oxidative stress, and disrupt immune function. Building studies show that nearly half of U.S. homes and schools have signs of water damage and mold (6).

Case example: A 29-year-old patient presented with relentless fatigue, unrefreshing sleep, and frequent sinus infections. Standard labs were unremarkable. Urine testing revealed elevated ochratoxin A.  Sinus testing using next generation sequencing identified several species of mold, fungus and bacteria that needed to be treated.  After home remediation, sauna therapy, glutathione support, and mast cell stabilization, her energy gradually returned.

2. Infections (Tick-Borne and Viral)

Chronic infections like Lyme, Bartonella, and Babesia, along with viral reactivations (EBV, HHV-6), are strongly linked to fatigue syndromes (7,8). These infections:

  • Suppress mitochondrial function.
  • Trigger the CDR persistently.
  • Confuse the immune system, leading to inflammation and autoimmunity.
  • Disruption of nervous systems including central sensitization and autonomic nervous system dysfunction

Babesia, as a red blood cell parasite, similar to malaria, can directly damage red blood cells, impairing oxygen delivery and further contributing to fatigue (9).  Research on long COVID echoes the same mechanisms: lingering viral fragments keep the immune system “on guard,” leaving mitochondria in defense mode (10).

3. Environmental Toxicants

Toxins such as pesticides, heavy metals, and PFAS accumulate over decades. These disrupt hormonal signaling, mitochondrial enzymes, and detox pathways (11,12). Even low-level exposures, when layered over infections or stress, can tip the body into exhaustion.

Current research does not show a direct association between exposure to many other chemical toxicants like: pesticides, parabens, phthalates, bisphenols, or glyphosate and fatigue in humans.  However, these chemicals are linked to other health problems, including endocrine disruption, metabolic disturbances (such as obesity and diabetes), and reproductive health issues, which may sometimes involve fatigue, although no studies specifically identify fatigue as a direct outcome of exposure. (20, 21, 22)

Unforgetting lens: Environmental toxins remind us how deeply interconnected we are with our world. Fatigue, in this context, becomes the body’s protest — a call to cleanse not only personal terrain but the collective one we share.

4. Nervous System Dysregulation

The autonomic nervous system plays a profound role in fatigue. Chronic stress, trauma, or limbic system sensitization can keep the body stuck in survival physiology. Research shows vagus nerve dysfunction contributes to poor sleep, impaired digestion, and heightened inflammation (13,14).

Patients often describe:

  • Feeling “wired but tired.”
  • Insomnia despite exhaustion.
  • Flare-ups after emotional stress.

Healing requires more than biochemistry — it requires nervous system retraining: vagus nerve exercises, somatic therapy, breathwork, and tools like BrainTap or Apollo Neuro.

5. The Overlooked Role of EMFs

Electromagnetic fields (EMFs) are biologically active, increasing oxidative stress and disrupting melatonin, the hormone that regulates sleep and repair (15,16). For sensitive patients, fatigue often worsens after heavy digital exposure. Reducing nighttime EMF load (turning off WiFi, grounding devices) can make a measurable difference.

 

The Cost of Misinterpretation

When fatigue is seen only as weakness or depression:

  • Patients push harder, leading to more crashes, longer recovery times and diminished resilience.
  • They cycle through stimulant medications that mask symptoms without solving them.
  • They lose trust in their body, seeing it as broken or lazy.

This is the danger of ignoring the body’s “check engine light.”

 

The Unforgetting Perspective

Fatigue, through the lens of The Unforgetting Project, is not a defect. It is the body’s insistence:
